Service Animal Application

S.E.R.V.E. is proud to partner with A Pawsitive Approach Dog Training to provide fully trained Service Animals to Veterans and their immediate family members in the greater gulf coast area. In order to qualify, there must be a Veteran in the household and recipients can either be a Veteran, a Veteran Spouse or child of a Veteran. Additionally,

You must be able to participate in personal training sessions with the trainer in public places and have transportation.
This program requires a focused dedication from you, your family, the service dog, and the trainer. This dedicated commitment, at minimum, is around 20 hours per week when actively training and can go as high as 30 hours when necessary. Most of the training is scheduled Monday-Friday with group attendance scheduled on the weekends.
You must be financially stable, stably housed, and financially capable of providing all expenses and care for your own puppy/Service Dog.

LIABILITY AND RESPONSIBILITY:

Applicant agrees to maintain the Service Animal in a safe, sanitary, and humane environment; to provide proper nutrition, shelter, exercise, and routine veterinary care; and to ensure compliance with all federal, state, and local laws. Applicant assumes full legal responsibility and liability for any damages, injuries, or losses caused by the Service Animal following placement.

RETURN OF ANIMAL PROVISION:

Failure to maintain the Service Animal’s health, safety, training standards, or humane treatment shall constitute material breach of agreement. Upon written notice by S.E.R.V.E., Applicant agrees to immediately surrender and return the Service Animal to S.E.R.V.E. Failure to comply may result in legal action to recover the animal and associated costs.
